Sacramental Record Request

- Certificates are only available for sacraments received at St. Elizabeth Ann Seton in Plano, Texas.
  All Sacramental Records must be accompanied by a written request, using the form below.
  All requests may only be made or picked up by the person who received the sacrament or their parent or guardian.
  Sacramental Records can be mailed directly to you or to another parish, or picked up in person.
  
  Sacramental Records can be picked up in person at our Parish Office, located at 2701 Piedra Drive, Plano, TX 75023, during regular business hours: Monday to Friday, 9:00 am–12:00 pm and 1:00 pm–5:00 pm (excluding holidays). You will be notified by email when the certificate is ready for pickup.
